For married applicants, spouse should comply the following:

  • Three (3) photocopies of Primary Valid ID of Attorney-in-Fact w/ 3 specimen signature – if Applicable only

Note: Special Power of Attorney for OFW spouse must be notarized prior to date of departure. If currently abroad, SPA must be notarized by a Philippine Consular Officer or notarized by a local notary of the country where the member is working but duly authenticated by the Philippine Consulate.

Locally Employed

For Private Employee:
  1. Completely filled out Housing Loan Application (HLA)
  2. Income Documents
    • Notarized Certificate of Employment and Compensation (CEC)
    • Payslips for the last three (3) months
For Gov’t Employee:
  1. Attached Certified One (1) month payslip/payroll
  2. Photocopy of Latest Income Tax Return (ITR)
  3. Completely filled out Membership Status Verification Slip (MSVS)
    (must not have IN DEFAULT housing loan and no Arrears in other Pag-ibig Loans)
  4. Photocopy of Member’s Data Form (MDF) from Pag-IBIG
  5. Photocopy of Total Monthly Contribution from Pag-IBIG (must have at least 24 months contribution)
  6. Photocopy of Lump Sum Payment Receipt – if Applicable only
  7. Three (3) photocopies of Primary Valid ID w/ 3 specimen signature
  8. Photocopy of TIN Verification duly verified by BIR (for  married women, last name must be in spouse’s name)
  9. Two (2) pcs 1×1 pic  (white background)

    • Official Receipt of Tax payment from bank supported with DTI Registration and Mayor’s Permit/Business Permit.
  3. Completely filled out Membership Status Verification Slip (MSVS)
    (must not have IN DEFAULT housing loan and no Arrears in other Pag-ibig Loans)
  4. Photocopy of Member’s Data Form (MDF) from Pag-IBIG
  5. Photocopy of Total Monthly Contribution from Pag-IBIG (must have at least 24 months contribution)
  6. Photocopy of Lump Sum Payment Receipt – if Applicable only
  7. Three (3) photocopies of Primary Valid ID w/ 3 specimen signature

    • Scanned copy of Employment Contract or POEA Standard Contract
    • Certificate of Employment and Compensation written on the employer/company’s Official Letterhead or
    • Certificate of Employment signed by employer (for household staff and similarly situated employees) supported by a photocopy of the employer’s ID or passport
  3. Iqama or resident ID
  4. Completely filled out Membership Status Verification Slip (MSVS)
    (must not have IN DEFAULT housing loan and no Arrears in other Pag-ibig Loans)
  5. Photocopy of Member’s Data Form (MDF) from Pag-IBIG
  6. Photocopy of Total Monthly Contribution from Pag-IBIG (must have at least 24 months contribution)
